JOB DESCRIPTION

Being a Home Support Worker is a rewarding career in which you will be able to build meaningful relationships with patients and their families and make a real difference in their lives. In this role, you will assist patients with their basic personal care and help them manage day-to-day living activities so that they can continue living at home while giving care givers and family members respite from the demands of caring for aging parents or loved ones. As a Spectrum HSW, you will provide our patients with the best possible care and support.

RESPONSIBILITIES:

1. Provide Basic Personal Care:
Under the direction of a Home Support Supervisor, deliver essential personal care to medically stable patients, ensuring their comfort and well-being.
Assist patients with tasks such as bathing, dressing, grooming, toileting, and skincare, while respecting their dignity and privacy.

2. Assist with Activities of Daily Living (ADLs):
Support patients in performing activities necessary for daily life, including mobility assistance, transferring, and positioning.
Engage patients in socialization activities to promote mental stimulation and emotional well-being.

3. Meal Preparation and Light Housekeeping:
Prepare basic meals and snacks according to patients’ dietary requirements and preferences, ensuring nutritional adequacy.
Perform light housekeeping tasks, such as washing dishes, taking out the garbage, and doing the patient’s laundry, to maintain a clean and hygienic living environment.
Conduct routine cleaning tasks, such as sweeping and vacuuming, within the patient’s immediate living area to ensure a safe and comfortable space.

4. Provide Respite Care:
Offer respite care to medically stable patients in the absence of their primary caregiver, providing companionship and assistance as needed.
Monitor patients’ well-being and respond promptly to any changes in their condition or needs.

5. Maintain Awareness of Patient’s Needs:
Stay informed about the patient’s abilities, medical conditions, treatment plans, dietary restrictions, personal interests, and any special needs or preferences.
Communicate effectively with healthcare professionals and family members to ensure continuity of care and support.

6. Travel Between Patients’ Residences or Facilities:
Requires flexibility and willingness to travel within the assigned region to provide home support services to patients in their residences or facilities.
Manage time effectively to adhere to scheduled appointments and minimize disruptions to patients’ routines.

QUALIFICATIONS

Supportive Care Worker Certificate, Health Care Aide Certificate or completion of Term 1 of a PSW program
Valid Vulnerable Sector Police Check and TB Test (within 6 months) as a condition of employment
Current CPR and First Aid Certificate
Effective English communication skills, written and verbal
Ability to interact with others in a caring, tactful and courteous manner
